To ensure we provide you with safe and appropriate care, you will answer a few questions regarding your health and history when submitted your case on DocBright.
Your information will be kept confidential between you and our Care Team, and will be used only to make sure we provide you with the best care possible.
Telehealth is not a replacement for primary care or regular exams. We encourage all patients to have a primary care physician, specialists as needed, and to have routine exams at least once per year.
When you begin your telehealth experience, you are also consenting to our Telehealth Informed Consent & your provider's Notice of Privacy Practices.
